Primary care profiling

Our Diabetes Primary Care Profiling Tool (DPCPT) uses local data to drive local improvement in primary care processes and outcomes.

Primary care practitioners play a pivotal role in the health care system and the DPCPT supports our clients in understanding the current baseline of activity. The profiling tool we have developed provides an accurate snapshot of primary care diabetes population. This provides individual practices with accurate data and can be aggregated into larger locality or CCG profiles.

The tool is designed to identify those most at risk of diabetes related complications, the level of diabetes control and care processes alongside identifying people who are at an increased risk of a diabetes diagnosis. The tool’s parameters can be customised for your areas to provide a unique and locally tailored snapshot. The DPCPT also identifies local variation within primary care. To reduce this variation we offer a best practice modelling programme.
